Mending little hearts ‘for life’? : promises of a cure and experiences of chronicity in a context of unequal access to paediatric heart surgery
Scientific paper ArODES

Medicine Anthropology Theory,  2025, vol. 12, 2, 1-27

Link to the publication

Summary:

Congenital heart defects (CHDs) are the most common type of major birth defects worldwide. Yet globally, access to high quality treatment is very limited and uneven with most patients living in places without adequate diagnostic or treatment. Based on ethnographic engagement with Beninese and Togolese children undergoing surgical treatment in Switzerland through a humanitarian medicine programme, this paper explores the multiple temporalities and experiences of chronicity at play in the lived experiences of families with children with CHDs in a context of profound health inequalities. These temporal experiences encompass the various promises of a cure made to them, ensuring continued investment in their child’s health, experiencing a sense of rebirth, and navigating the potential risks of future complications. The article highlights how families facing CHDs in underserved regions encounter distinct forms of chronicity compared to those in more privileged areas. It identifies four kinds of chronicity in the families’ lived experiences: symptom-related, procedural, follow-up, and emotions-related.

Scientific paper

Summary:

Scientific paper

Vaucher Carla

Sciences sociales et santé, 2022 , no  40, pp.  37-60

Link to the publication

Summary:

Summary:

Partnership with social care professionals as a context for promoting self-determination among people with intellectual disabilities
Scientific paper ArODES

Research in developmental disabilities,  2020, vol. 99, pp. 1-10

Link to the publication

Summary:

Background : People with intellectual disabilities who live in residential facilities may need social support to express self determination. Relationships with social care professionals provide an important context for promoting self-determination. Adopting a socioecological perspective, our study aimed to better understand the nature of these relationships. Method : Over a period of ten months, we held 13 focus group discussions with a total of 20 participants, including both residents and staff at facilities for people with intellectual disabilities. Using transcripts of these discussions, we analysed expressions of self-determination among people with intellectual disabilities and the responses of social care professionals. Results : Our results highlight the importance of relational adjustment in fostering self-determination among people with intellectual disabilities and underline the importance of respective roles within relationships between people with intellectual disabilities and social care professionals. Conclusion : The partnership between people with intellectual disabilities and professionals seems to be the most effective type of relationship in order to support the self-determination of people with intellectual disabilities. This paper provides a fresh perspective on the role played by people with intellectual disabilities in their relationships with social care professionals. By engaging people with intellectual disabilities as partners in fostering self-determination, social care professionals can encourage social participation and feelings of empowerment. Relationships based on partnership offer people with intellectual disabilities a form of hetero-regulation that can help them overcome challenges to behaving in a fully self-determined way. However, partnership also requires changes in professional practices and attitudes.

Environmental, personal, and relational barriers and facilitators to self-determination among adults with intellectual disabilities
Scientific paper ArODES

Scandinavian journal of disability research,  2020, vol. 22, no.1, pp. 97-107

Link to the publication

Summary:

For people with intellectual disabilities, the potential for self-determination depends on both personal characteristics and factors related to the social environment. This study examines barriers and facilitators to self-determination among residents at facilities for people with intellectual disabilities. We held 13 focus group discussions to explore the opinions and experiences of 10 people with mild to moderate intellectual disabilities and 10 social care professionals at three residential facilities in Western Switzerland. We identified environmental, personal, and relational barriers and facilitators to self-determination associated with attitudes, representations, and institutional settings. Within the social environment of people with intellectual disabilities, relationships based on trust, partnership, and collaboration appear to promote self-determined behaviours. Ideally, strategies for promoting self-determination among people with intellectual disabilities should take both personal characteristics and the social environment into account.

Scientific paper

Vaucher Carla

Revue des sciences sociales, 2020 , no  63, pp.  112-119

Link to the publication

Summary:

Knowledge translation and evidence-based practice : a qualitative study on clinical dietitians' perceptions and practices in Switzerland
Scientific paper ArODES

Journal of the academy of nutrition and dietetics,  2019, vol. 119, no. 11, pp. 1882-1889

Link to the publication

Summary:

Background : Knowledge translation (KT) in health care is essential to promote quality of care and reduce the knowledge-to-practice gap. Little is known about KT among dietitians, and a better understanding of how this process pans out is fundamental to support their clinical practice. Objective : To explore clinical dietitians’ perceptions and practices concerning preferences and access to information sources in clinical practice, KT activities, research in nutrition and dietetics, and evidence-based practice (EBP). Design, participants, and setting : Eight interviews and two focus groups involving a total of 15 participants were conducted in 2013 among members of the Swiss Association for Registered Dietitians in the French- and German-speaking regions of Switzerland. Analysis performed : Thematic analysis drawn from a constructivist grounded theory approach. Results : Information from colleagues and experts of the field were favored when facing unfamiliar situations in clinical practice. Critically selecting evidence-based information was considered challenging, but dietitians declared they were at ease to integrate patients’ preferences and values, and their clinical expertise and judgment, in decision making, which are fundamental elements of EBP. A major reported barrier to KT was the perception that time to identify and read scientific literature was not expected during working hours and that instead, this time should be spent in clinical activities with patients. On the other hand, dietitians identified that their frequent involvement in educational activities such as knowledge dissemination or tailoring favored the integration of evidence into practice. Finally, dietitians struggled more to identify evidence-based information about counseling and communication than about biomedical knowledge. Conclusions : Dietitians mentioned being involved in each step of the KT process (ie, synthesis, dissemination, exchange, and ethically sound application of knowledge). Barriers and facilitators identified in this study need to be explored in a larger population to develop strategies to facilitate KT and EBP in dietetics practice.

The importance of the relational needs of people with learning disabilities in the promotion of self-determination
Scientific paper ArODES

Annick Cudré-Mauroux, Geneviève Piérart, Carla Vaucher

British journal of learning disabilities,  2019, vol. 47, no. 3, pp. 174-180

Link to the publication

Summary:

People with learning disabilities may need to receive support from social care professionals to make choices and to change things in their life. They may need to receive advice, but sometimes they prefer not to receive advice. Professionals must trust them when they try something new. It is important that professionals know what people with learning disabilities want and need to support them in a better way. It is important that they become aware of the importance to express what they want and what they need. Abstract: Background: Intervention to increase self‐determination of people with learning disabilities is a major issue for social care practices. This study examines relational needs of people with learning disabilities regarding self‐determination in the context of the relationships they share with social care professionals. Method: We held focus group discussions to explore the relational needs of 10 residents and 10 educators at three facilities for people with learning disabilities located in the French part of Switzerland. Results: Relational needs expressed by participants with learning disabilities concern three distinct stages of the self‐determination process (pre‐choice, post‐choice and goal achievement). Conclusion: They suggest the importance of the relational quality of interventions aimed to enhance self‐determination of people with learning disabilities.

Summary:

This study examines perceptions and understandings of self-determination in the context of relationships between people with intellectual disabilities and social care professionals. We held focus group discussions to explore the views and experiences of 10 residents and 10 professionals at three facilities for people with intellectual disabilities located in Western Switzerland. Participants perceived and understood self-determination in terms of decision-making, social skills, procedures, identity, self-consciousness, autonomy, freedom, barriers, and facilitators. The research process highlighted the shifting and situational nature of the concept, as well as the importance of self-determination for people with intellectual disabilities. The findings also highlight the importance of discussion and reflection on the concept of self-determination and its benefits for people with intellectual disabilities.
