Summary:

Hintergrund: Die COVID-19-Pandemie hatte erhebliche Auswirkungen auf die Gesundheitsversorgung. Pflegende spielten eine entscheidende Rolle zur Bewältigung der Krise. Ziel: Zielsetzung der Studie ist die Untersuchung des physischen und psychischen Gesundheitszustands sowie der Ressourcen von diplomierten Pflegenden während der Pandemie. Methoden: In einer gesamtschweizerisch durchgeführten Längsschnittstudie mit Mixed Methods Design erfolgte die Datenerhebung bei Pflegenden in Deutschschweizer Akutspitälern zwischen 2021 und 2022 durch Fragebogenerhebungen und Fokusgruppen-Interviews. Die Daten wurden mit multipler Regressionsanalyse sowie Knowledge-Mapping und Neuman-System-Modell ausgewertet. Die Ergebnisse: Ergebnisse zeigen, dass sich die Gesundheit, Lebensqualität und Zufriedenheit am Arbeitsplatz verbessern und der Stress abnimmt. Dabei sind beeinflussende extrapersonelle Faktoren, beispielsweise Reorganisationen mit aktivem Einbezug von Pflegenden. Auf der interpersonellen Ebene ist etwa die Unterstützung durch Vorgesetzte oder Kolleginnen und Kollegen hilfreich. Auf der intrapersonellen Ebene war unter anderem die Anerkennung der Fähigkeiten der Pflegenden wichtig, um während der Dauer der COVID-19-Pandemie gesund zu bleiben. Schlussfolgerungen: Entscheidend ist, vermehrte Wertschätzung, Partizipation, Ausbildungs- und Karrieremöglichkeiten sowie flexible Arbeitsmodelle für eine nachhaltige Verbesserung der Situation, welche sich schlussendlich auf Gesundheit und Wohlbefinden der Pflegenden auswirkt, langfristig in die Gesundheitsversorgung zu integrieren.

Background : Students are exposed to high demands, which can have an impact on their health. The COVID 19 pandemic has additionally increased the psychological burden through social isolation. A low-threshold assessment of the individually experienced state of health as well as information on health support services is necessary to support students in their health behaviour. Method : Questionnaire survey and focus group interviews to assess the individually experienced state of health of students recruited from the CoSaMo health clinic of the Freiburg University of Applied Sciences (HEdS-FR) to develop a needs-oriented smartphone application in French and German. Results : A total of 37 out of 71 students participated in the survey (06.2021-01.2022), 15 of whom also took part in a focus group interview. The study survey had a good response rate of 52%. The following topics were explored in more depth with the students for implementation in a smartphone application: Stress, Nutrition, Physical Activity, Substance Use, Internet Use, Sexuality, Violence, General Health and Wellbeing. Students are aware of health issues and expect a quick recording of their current health status and easily accessible information on support services. Discussion : The sample shows that stressful situations related to studies, family and finances can affect general well-being, especially when several factors come together. The psychological stress is higher than the physical demands, which can manifest itself in the form of depressive mood. Students rely on multiple sources of income to finance their studies and living expenses. Students are challenged by their studies and their personal everyday life and are interested in quickly finding access to up-to-date health information and support that is oriented towards their individual needs. Conclusions : The smartphone application MonCoSaMo is an easy-to-use tool for recording individual health status and providing information on health support services. A broader survey is necessary to obtain more meaningful results for a larger student population.

Use of physical restraints in nursing homes: a multicentre cross-sectional study

Summary:

Background

Although many countries have implemented strict legal rules, the prevalence of physical restraints in nursing homes seems to remain high. In Switzerland, data related to the frequency of physical restraints are scarce and little is known about associations with resident and nursing home characteristics. The aim of this study was to investigate the prevalence and types of physical restraints in nursing homes in two Swiss cantons and to explore whether resident-related and organisational factors are associated with the use of physical restraints.

Methods

We conducted a multicentre cross-sectional study. Twenty nursing homes with 1362 residents from two culturally different cantons were included. Data on physical restraints and residents’ characteristics were extracted from residents’ records (11/2013 to 2/2014). Organisational data were collected by questionnaires addressing nursing home directors or nursing managers. Sample size calculation and outcome analysis took cluster-adjustment into account. Descriptive statistics and multiple logistic regression analysis with nursing homes as random effect were used for investigation.

Results

The prevalence of residents with at least one physical restraint was 26.8 % (95 % confidence interval [CI] 19.8–33.8). Centre prevalence ranged from 2.6 to 61.2 %. Bilateral bedrails were most frequently used (20.3 %, 95 % CI 13.5–27.1). Length of residence, degrees of care dependency and mobility limitation were significantly positively associated with the use of physical restraint, but none of the organisational characteristics was significantly associated.

Conclusion

Approximately a quarter of the nursing home residents included in our study experienced physical restraints. Since variation between nursing homes was pronounced, it seems to be worthwhile to explore nursing homes with particularly low and high use of physical restraints in future research, especially by using qualitative methods. There is a need for effective interventions aiming at restraint-free nursing care. Development of interventional approaches should consider specific residents’ characteristics associated with restraint use.
