Models of Integrated Acute Care for Older Adult Inpatients That Incorporate Integrative Health: An Integrative Review

Background: The use of integrated acute care for older adult inpatients is a growing field, especially the use of integrative health-care practices for managing complex, chronic, age-related health conditions. Scientific evidence suggests that these practices should be incorporated into older adult inpatients’ daily care.
Aim: Conduct an integrative review of studies on integrated acute-care models for older adult inpatients that incorporate integrative health services.
Methods: We searched Medline Ovid ALL, Embase.com, CINAHL, APA PsycINFO Ovid, Web of Science Core Collection, ProQuest Dissertations & Theses A&I, Cochrane Library, and CAMBase bibliographic databases for studies, published between 1990 and 2023, on integrated acute-care models for older adult inpatients that incorporated integrative health services. The search associated the domains of acute care, geriatrics, internal medicine, rehabilitation, hospitalization, geriatric psychiatry, integrated/integrative care, care models, practices and coordination, interprofessionalism and multidisciplinarity, collaborative practices, and complementary therapies. The review was completed in June 2024.
Results: We retained 32 studies conducted in North America, Europe, Australia, and Asia, including 46,899 older adult inpatients, 39 physicians, 148 nurses, 695 allied health-care professionals, and 358 informal caregivers. Three integrated acute care models were identified: the Acute Care for Elders model, the Integrated General Hospital model, and the Transitional Care model. Three integrated acute psychogeriatric-care models were identified: the Admiral Nursing model, the Lewy body dementia Admiral nursing service model, and the Care for Acute Mentally Infirm Elders model. A single, hybrid, Integrated, People-Centred Health Services model for acute and community health care was identified. We found the Scaling Integrated Care in Context model for measuring integrated care development within health-care systems.
Conclusion: Few studies have investigated integrated acute-care models incorporating integrative health services for older adult inpatients. Existing acute-care models including integrative medicine should be explored further, and new, more inclusive models should be developed.

Background: Climate change is a health emergency. Each year, it is estimated to cost more than 230 million years of life expectancy, with 4–9 million premature deaths associated with air pollution, and 9 million excess deaths due to non-optimal temperatures, representing 7% more temperature-related deaths since 2015 and 66% more since 2000. Objective: Identify and evaluate the reliability, fidelity, and validity of instruments measuring nurses’ knowledge and awareness of climate change and climate-associated diseases. Methods: A systematic literature review will retrieve and assess studies examining instruments measuring nurses’ knowledge and awareness of climate change and climate-associated diseases. Using predefined search terms for nurses, climate change, literacy and scales or tools, we will search for published articles recorded in the following electronic databases, with no language or date restrictions, from their inception until 31 October 2023: Medline Ovid SP (from 1946), PubMed (NOT Medline[sb], from 1996), Embase.com (from 1947), CINAHL Ebesco (from 1937), the Cochrane Library Wiley (from 1992), Web of Science Core Collection (from 1900), the Trip Database (from 1997), JBI OVID SP (from 1998), and the GreenFILE EBSCO. We will also hand-search relevant articles’ bibliographies and search for unpublished studies using Google Scholar, ProQuest Dissertations and Theses Global, and DART-EUrope.eu. This will be completed by exploring the gray literature in OpenGrey and the Grey Literature Report, from inception until 31 October 2023, in collaboration with a librarian. Twelve bibliographic databases will be searched for publications up to 31 October 2023. The papers selected will be assessed for their quality. Results: The electronic database searches were completed in May 2023. Retrieved articles are being screened, and the study will be completed by October 2023. After removing duplicates, our search strategy has retrieved 3449 references. Conclusions: This systematic review will provide specific knowledge about instruments to measure nurses’ knowledge, awareness, motivation, attitudes, behaviors, beliefs, skills, and competencies regarding climate change and climate-associated diseases.

Résumé:

Current data and scientific predictions about the consequences of climate change are accurate in suggesting disaster. Since 2019, climate change has become a threat to human health, and major consequences on health and health systems are already observed. Climate change is a central concern for the nursing discipline, even though nursing theorists’ understanding of the environment has led to problematic gaps that impact the current context. Today, nursing discipline is facing new challenges. Nurses are strategically placed to respond to the impacts of climate change through their practice, research, and training in developing, implementing, and sustaining innovation towards climate change mitigation and adaptation. It is urgent for them to adapt their practice to this reality to become agents of change.

Création d’un prototype de jeu sérieux sur la gestion des données de la recherche

Les professionnel-le-s de l'information travaillant en bibliothèque académique sont désormais régulièrement amené-e-s à accompagner des chercheurs et chercheuses dans la gestion de leurs données de recherche. Dans le but d'offrir à ces professionnel-le-s une formation introductive sur cette thématique, nous avons conçu et testé un prototype de jeu sérieux. Ce projet a été réalisé dans le cadre du Master en Sciences de l'information à la Haute école de gestion de Genève. Méthode : Notre méthodologie s’inspire de deux modèles de conception de jeux sérieux. Elle s'articule en trois étapes : (1) la phase de définition, qui présente les résultats de l'analyse des besoins, l'exploration de l'existant et le dispositif de formation ; (2) la production du prototype et les tests effectués ; (3) l'accompagnement et l'évaluation. Résultats : Le jeu proposé, « Mission GDR : ultime quête avant les fêtes » est inspiré de la série de jeux d'évasion en boîte « Unlock ». Il s'agit d'un jeu collaboratif pour 1 à 4 participant-e-s dont l'objectif est de compléter une feuille de route représentant le cycle de vie des données et les tâches qui s'y rapportent. Cette feuille, accompagnée d'un guide complémentaire rappelant la théorie abordée dans le jeu, peut ensuite être réutilisée comme ressource dans la pratique professionnelle des joueuses et joueurs. Discussion : Les tests effectués auprès de personnes aux profils divers démontrent que le jeu est apprécié pour ses aspects ludiques tout en offrant un apport théorique introductif. Initialement prévu pour les professionnel-le-s de l'informations, il pourrait également être proposé à d'autres personnes concernées par la gestion des données, notamment des équipes de recherche. Conçu pour être indépendant d’une session de formation, il est cependant tout à fait envisageable de l’intégrer dans un tel programme.

Effectiveness of educational interventions to increase skills inevidence-based practice among nurses :

Background: Using evidence-based practice (EBP) improves the implementation of safe, high-quality healthcare for patients, reduces avoidable costs, and plays a crucial role in bridging knowledge–action gaps and reducing health inequities. EBP combines the best available evidence in the relevant literature with patient preferences and values and healthcare professionals’ (HCPs) expertise. Methods: Systematic searches of ten bibliographic databases, unpublished works, and the Grey Literature Report sought studies published up to 30 September 2022. Results: The 15 studies retained involved 2712 nurses. Three types of effective educational interventions were identified: (1) multifaceted educational strategies incorporating mentoring and tutoring; (2) single educational strategies, often delivered online; and (3) multifaceted educational strategies using the five steps of EBP. Eleven primary outcomes (EBP beliefs, EBP self-efficacy, perceived EBP implementation, EBP competencies, EBP knowledge, EBP skills, EBP attitudes, EBP behaviors, EBP desire, EBP practice, and perceptions of organizational culture and readiness) were assessed using 13 qualitative and quantitative instruments. Conclusions: Ensuring the successful implementation of EBP requires effective educational strategies. Computer-based learning seems the most cost-effective and efficient strategy, when considering caregivers’ characteristics, the clinical field, and educational interventions across the pre-, peri-, and post-implementation processes.

Background: The implementation of evidence-based practice (EBP) in daily health care practice is strongly encouraged; it is widely recognized as a means to improve the quality and safety of health care for patients and reduce avoidable costs. Primary care nurses and physiotherapists face numerous challenges in trying to ensure that they deliver effective daily care. Broadly promoted educational interventions aim to increase the integration and implementation of EBP in their daily practice. Objective: This systematic review will retrieve and evaluate publications examining the effectiveness of educational interventions to increase the integration and implementation of EBP among nurses, nurse practitioners, and physiotherapists active in primary care. Methods: We will conduct a systematic review of published articles in relevant professional, scientific journals (from their start dates) and in the following electronic databases, from inception until October 31, 2020: Medline Ovid SP (from 1946), PubMed (NOT Medline[sb]; from 1996), Embase.com (from 1947), CINAHL Ebesco (from 1937), the Cochrane Central Register of Controlled Trials Wiley (from 1992), PsycINFO Ovid SP (from 1806), Web of Science Core collection (from 1900), PEDro (from 1999), the JBI Database of Systematic Reviews and Implementation Reports (from 1998), and the Trip Database (from 1997). We will use the predefined search terms of “evidence-based practice,” “nurses,” or “physiotherapists” and combinations with other terms, such as “educational interventions.” We will also conduct a hand search of the bibliographies of all the relevant articles and a search for unpublished studies using Google Scholar, the ProQuest Dissertations and Theses dissemination, Mednar, WorldCat, OpenGrey, and Grey Literature Report. We will consider publications in English, French, German, and Portuguese. Results: The electronic database searches were completed in October 2020. Retrieved articles are currently being screened, and the entire study is expected to be completed by March 2021. Conclusions: This systematic review will provide specific knowledge about the effectiveness of educational interventions to increase the implementation and integration of EBP in the daily practice of nurses and physiotherapists providing primary care services. Its findings will inform us about the types and frequencies of the most successful educational interventions.
